His nurse tells him stories of Old Narnia, when animals talked and the trees were awake, and Caspian longs for those days to return, but they seem to be only the stuff of legend. When Miraz learns of the tales that Caspian's nurse has been telling him, she is removed and a tutor, Doctor Cornelius, is hired for the young prince. But Doctor Cornelius is also an advocate of Old Narnia. One night, Doctor Cornelius wakes Caspian and tells him that Queen Prunaprismia has had a son, and the doctor believes Miraz intends to kill Caspian so that his own son could be king. Cornelius gives Queen Susan's enchanted horn to Caspian and tells him to ride to Archenland where he will be safe. As Caspian travels through the forest, it begins to storm and he is knocked unconscious by a falling branch. When he awakens, he finds himself being helped by Old Narnians — the Talking Badger, Trufflehunter, and two Dwarfs, Trumpkin and Nikabrik. After hearing his story, they accept him as their king, and he leads them to war against Miraz and the Telmarines. When the Old Narnians have lost a series of battles and morale is low, Caspian blows Queen Susan's enchanted horn to call for aid. The horn pulls the four Pevensies back into Narnia. When the Pevensies finally reach Caspian at Aslan's How, High King Peter suggests that he send Miraz a challenge to single combat to fight for Caspian's right to the throne. Caspian fights at the second Battle of Beruna and takes part in the victory over the Telmarines. Caspian is eventually crowned King of Narnia, although, being humble, he does not feel himself sufficient.

When Edmund, Lucy, and their cousin Eustace are sent by Aslan to help with the quest, Caspian welcomes them to his ship, the Dawn Treader. While on the Lone Islands, the four of them and Reepicheep decide to walk across Felimath and are captured by slavers. Caspian is purchased by the Lord Bern, one of the seven missing lords, and together they free the others and abolish the slave trade in the Lone Islands. On Dragon Island, Caspian recognizes the Lord Octesian's arm-ring on the dragon's (Eustace's) arm. When Aslan appears to them on Deathwater Island, Caspian and Edmund forget their quarrel over the water that transforms objects into gold. Caspian's courage is tested in the Land of the Duffers, where he chooses to meet the invisible Dufflepuds rather than try to escape from them in secret. As the voyage continues, Reepicheep persuades Caspian to enter the Dark Island. When they finally reach the world's end, Caspian wants to go with Reepicheep to Aslan's Country, but Aslan appears to him and tells him that he must return to his country. Caspian returns to Ramandu's Island, marries his daughter, and sails back to Narnia in obedience to Aslan.

When Lord Drinian comes to him and says he feels responsible for Rilian's disappearance, Caspian raises an axe to kill him. But then he throws it away and says, "I have lost my queen and my son: shall I lose my friend also?" Ten years later, seeking Aslan's advice before his death, Caspian sets out on a voyage to the Lone Islands, where it was rumored that the great Lion had been recently sighted. Caspian has not sailed far when Aslan appears to him and tells him that his long-lost son has been rescued and is awaiting him in Narnia. Caspian sails back to Narnia and sees his son one last time. Then his hand falls, and in the sight of all Narnia, King Caspian the tenth dies. But Eustace and Jill see him again as a young boy in Aslan's Country. Then, Aslan grants Caspian something he has always wanted: a glimpse of our world, Earth.
